New/Established Patient Exam = $120 
Pediatric* New/Established Patient Exam = $75
This is an hour appointment. At this visit, we will do an initial history, thorough review of systems, a series of complaint specific neurological, orthopedic, and physical exams, as well as movement analysis and an initial treatment.

30-min Follow-up Appointment = $60
60-min Follow-up Appointment =  $120
Follow-ups are in 30 or 60-minute blocks.

Pediatric* Follow-up Appointment = $45
Pediatric follow-ups are in 30 minute blocks.

*Pediatric is defined as 15 years of age and under

Do you accept insurance?

We do not accept insurance at Pre-Form Movement.

Where is your entrance?

Accepted Answer

We are located in the Gettysburg Shopping Center. When you park, enter the bright blue door located under all of the business signs. Once you enter, follow the L-shaped hallway to the end and then enter the bright green door. Lastly, you will enter a white door with "Pre-Form Movement" overhead. The waiting room is straight ahead.

The Middle Street entrance is locked, please use the parking lot entrance.
